We operate throughout the UK.
Our service will:
Provide realistic, tailor made packages of support and accommodation based on an individual’s needs. To include shared accommodation and parent and child placements.
To include self contained or shared accommodation or accommodation suitable for parent and child placements.
Provide quality accommodation and a chance to live independently with support, to enable young people to make informed choices about their lives and direction and to support young people through their mistakes.
Provide a stepped package of support. This can include shared homes some with higher levels of support and independent flats with lower support levels. Support can be increased and decreased according to need.
Provide high quality staff and a robust thorough training and supervision package to ensure staff provide good quality support.
Provide outcomes based independence training for young people that is relevant and bespoke. Moving On assists young people with the development of Life Skills, practical units in these life skills are accredited through the AQA unit award scheme.
Provide regular updates to placing workers and regular attendance at meetings to try and demonstrate progression, according to an individual’s care plan. Outcomes star are used to help demonstrate a young person’s journey of change and to help to focus keywork.
Provide a value for money service
Provide a 24 hour support line which is manned by experienced support staff.
Monitor outcomes at both individual and service level through Outcomes star, feedback forms and performance monitoring worksheets.
